Circle of Care is an independent non-profit organization dedicated to assisting Passaic County youth (up to age 21) and their families. Our goal is to help families attain positive outcomes for their children in their own homes and communities.
Once referred, your family will be paired with a Care Manager who will work alongside you to form a Child Family Team (CFT). This team includes both natural supports (like family, friends, and neighbors) and formal supports (such as teachers, therapists, and mentors). Together, the team will develop a personalized action plan tailored to your child’s needs.
Our goal is to create a comprehensive support system that addresses your child’s emotional, social, educational, and health needs, helping your family move forward with confidence.
We are child and family centered, with strengths, needs and preferences of the child, family, and team guiding the services developed.
Circle of Care services are community based, individually tailored to children and their families, flexible, and outcome based.
Circle of Care endeavors to be culturally competent, with services that are responsive to the culture, race, and ethnicity of the families we serve and the communities in which they live.
A Child-Family Team (CFT) is a group of people composed of “natural” (relatives, friends, neighbors, etc.) and “formal” (teachers, therapists, other professionals, etc.) supports who are involved with the family and play an important role in the child’s life. This team will help “wrap” supports and services around your family.
The COC Care Manager will partner with the CFT members to develop an agreed-upon Individual Service Plan (ISP) where each CFT member plays a part in ensuring the plan is successful.
The ISP will address all areas of the child’s life, including family interactions, safety concerns, emotional/psychological supports, educational objectives, legal issues, and medical concerns.
The youth is a key member of the Team and contributes to the development of their ISP. All team members play an important role in ensuring the success of the ISP along with the youth and the family.
The Team will evolve over time as the family’s relationships, interests, and needs change.
