PerformCare is the “access department” for NJ Children’s System of Care partners, like Circle of Care. When you call PerformCare, a trained and licensed Care Coordinator will ask you some basic questions to help determine if your family needs immediate assistance. You, as the parent/caregiver, must call PerformCare yourself, though someone may assist you when you do so.
Make sure you have ample time to speak with the Care Coordinator. Have details such as dates of birth, address, social security number (if the youth has one; services are available to all NJ residents), and any contacts such as a DCPP worker, therapist, or counselor at hand.
You will speak to a licensed clinician who will ask questions about your child’s needs.
Voluntary participation in our services at no cost to you, during which you and your child decide the resources and services being utilized.
Your child can remain safe at home, in school, and in their community.
A Care Manager will be assigned to your family to help you identify strengths, challenges, and strategies to alleviate those challenges with your values and preferences in mind. This will be done following the Wraparound Philosophy and includes the development of a Child-Family Team.
Ideally, children will work with COC and their CFT on a short-term basis, but the duration of services can vary depending on the child and family circumstances.
All services are confidential and coordinated around the families’ schedule/needs. Circle of Care staff are legally required to report all suspicion of child abuse and neglect.
